Make updates to my policy
During the seasons of life, sometimes changes need to be made on your insurance or investment policy. While some changes require that you work with an advisor or our Member Services team, others can be done on your own. The list below includes some commonly requested change forms. Please fill them out and return them to us via email (member.services@serenialife.ca) or by mail:

Change my Beneficiary:
Update your named beneficiary, kindly complete and sign the Revocable Beneficiary Designation form.
Update my Address:
If you have recently moved and wish to update your address, kindly complete the Address Change form.
Change my Name:
If your name has recently changed, please complete the Name Change form.
Update my Banking:
If you are currently paying your premium monthly via a Pre-authorized Debit agreement, and your banking has changed, please complete the Pre-authorized Debit Agreement form. We strongly encourage you to include a copy of a void cheque or direct deposit form from your bank to ensure the banking detail accuracy.
To make any other types of changes, please contact your Serenia Life advisor.
